Glass, sealed unit or the whole window
Most double-pane and triple-pane windows are built as sealed units: layers of glass held apart by a spacer and sealed around the edge. Fog between the panes usually means that seal has failed.
The right next step could be new glass, a new sealed unit or a new window. We cannot confirm which from photos alone, so your photos start the conversation and anything we cannot see is checked in person.
